School Programs

Diabetes Network of Arizona partners with schools to ensure students living with diabetes are supported, safe, and understood throughout the school day.

 

Our programs provide education, training, and advocacy to help school staff confidently manage diabetes care and create inclusive learning environments for students and families.

Request Supplies

Access to diabetes supplies can make a meaningful difference in daily care. This request form allows individuals and families to request essential diabetes supplies to help manage care more safely and confidently.

Our team reviews each request to help ensure support is provided where it is needed most.

Donate Supplies

Donating unused or unopened diabetes supplies helps ensure individuals and families across Arizona have access to essential resources. Your donation directly supports those navigating diabetes and helps reduce barriers to care.

Every contribution makes a meaningful impact within the community.

Early Detection & Screenings

Early detection plays an important role in identifying diabetes risk and supporting timely care. Through education, outreach, and community-based screenings, Diabetes Network of Arizona works to raise awareness and encourage proactive health decisions.

These efforts help individuals and families recognize symptoms earlier and connect with appropriate resources and support.

Drug Discount Card

Managing diabetes can be costly. The Drug Discount Card helps individuals and families access potential savings on prescription medications, making care more affordable and accessible.

This resource is available to anyone who may benefit from prescription cost assistance.

DNA Media Release Form

This form provides consent for the use of photos, videos, and other media captured during Diabetes Network of Arizona programs and events. Completing the form helps us share stories and experiences while respecting participant privacy.
